LWML SWD Committee Expense Voucher
(Retreats, Conventions, Helpshops)

LWML SWD Personnel Profile Form
(Interests & Special Skills)
This profile form helps district leaders identify LWML members with gifts, experience, and skills that may suit elected or appointed positions.
Completing it expresses willingness to serve and assists the district in matching volunteers with areas of need.
2024–2026 Biennium Participant Release Form
This form collects participant information and includes required acknowledgments, assumption of risk, and liability releases for those volunteering or attending LWML SWD activities.
It covers general safety, COVID-19 considerations, and photo permissions for the biennium.
